Fall Tackle Fees                  Onsite Registration at Tumblweed Rec

Now - April 16 - $135                                April 17      9am -noon
April 17 - May 14 - $155                           May 15       9am - noon
May 15 - June 11 - $175                           June 12        9am-noon
June 12 - July 15 - $200

All Tackle Players shall submit $125 Administration Fee upon registration 
(includes cost for a jersey, city lights, and operating expenses) 

The Following documents will be required by all players -
flag, tackle, and cheerleaders.
If you choose to register online, documents can be provided at any of the onsite registration times. 

Original Birth Certificate or Certified Copy 
( No Photocopies Accepted )( a copy will be kept for season )

Explanation: Age must be determined to ensure the safety of all participants and a review of an original birth certificate is necessary to prevent changes to the date of birth for the purpose of gaining a competitive edge by playing in a different age division. 


Proof of Residency 
(A current utility bill: water,  electric,  gas,  or other legal document )( a copy will be kept )

                Explanation:        In order to participate in the Central Arizona Youth Football League, we must determine that your child resides within our Association’s designated boundaries. Proof of residency is a factor in determining your child’s eligibility and must be confirmed before he/she will be assigned to play/cheer for a football team.   If you move at any time after signing your child up for the program and providing the proof of residency, you will have a choice to stay with the currently assigned team or to switch to the team in which you now reside.  This decision must be made prior to certification date.

Medical CLEARANCE FORM (from medical provider) AND Medical CONSENT form (from Parent)

                Explanation:        To show that your child is physically fit to participate in the program.  If a child didn’t receive a physical at the time of registration, a Clearance form must be completed by a Medical Doctor (MD), Physician’s Assistant (PA), a Doctor of Osteopathy (DO), or a Nurse Practitioner and present the proof of such on a provided Medical CLEARANCE form. Form MUST have the doctor’s name, address, and phone number STAMPED on it and the provider’s signature.  The participant will not  be allowed to begin practicing with the team until such proof , or form is provided.  Parents are also required to complete the Emergency Medical CONSENT Form, with medical history and signatures.


Report Card   ( Most recent COMPLETE school year )

                Explanation:    In order to participate in youth football and cheer, your child must be scholastically fit.

                                          A 70% or better grade average in necessary or written approval from his/her school is required.
